Welcome to Fort Leslie J. McNair Army Health Clinic. 

We are a one stop shop for all your healthcare needs.  We are currently serving the following Demographic:

-Active Duty Military

-Activated Reserve/National Guard

 

 Services Available

PERIODIC HEALTH ASSESSMENT: Army, Air Force, USMC and Navy

  • Call to schedule your Part 1 appointment.
  • If you are a Male 35 or older, or Female 40 or over you must Fast 12 hours prior to your appointment.
  • You must complete the on-line portion of your PHA by logging on to your AKO and clicking on the drop down box My Medical Readiness. It is Mandatory to complete this prior to your Part 2 appointment.
  • Part 2 is the review of your Part 1 Exam and on-line Periodic Health Assessment. This exam can only be reviewed by a PA/NP/MD.

PDHA / PDHRA:  Army, Air Force, USMC and Navy

Post Deployment Health Assessment (PDHA) And Post Deployment Health Risk Assessment (PDHRA)

·         Complete the on-line portion of PDHA/PDHRA by going on AKO and clicking on the drop down box: My Medical Readiness

·         Call to schedule an appointment at Physical Exams

Part 1 – Call to schedule your part 1 appointment. Be sure to Fast for 12 hours prior to your appointment.  The purpose of the Part 1 is to complete all the required forms, Lab test, X-Rays, EKG if required, Hearing and Vision Screening. Excluding the X-Ray, we can provide ALL part 1 services in our facility.

NOTE: For Retirement Physicals, we request that you complete the DD Form 2807-1 Report of Medical History and drop off at least 72 hours prior to your scheduled Part II.  This offers our physician an opportunity to accurately and diligently to a comprehensive Retirement physical.
